ATHLETIC ELIGIBILITY
Kennedy High School is a member of the Empire League and competes under CIF Southern Section rules. Eligibility rules apply to all games including practice, inter-school scrimmage, league, tournament, and play-off games.
In order to compete in interscholastic athletics, a student must:
· Be under nineteen years of age by August 31 of the current school year
· Be academically eligible per the Anaheim Union High School District
· Be residentially eligible per CIF
· Have not participated in any tryout or competition for a professional or collegiate team
· Not participate with outside teams of the same sport during that season of sport
· Thoroughly read the Athletic Policies, then print out, complete, and return the four (4) forms found below.

Physical Form

PRINT OUT, FILL OUT COMPLETELY, AND RETURN TO YOUR COACH. The physical must be signed by a medical doctor (MD) or doctor of osteopathy (DO) - NOT a chiropractor, registered nurse, or physician's assistant! Don't forget to sign the form just below the medical history. The physical for next year must be taken AFTER June 4, 2012.

Don't forget to fill out the insurance information, including the policy number. If you do not have private health insurance, you will need to purchase coverage through the school. Fill out both sides of this form so that they are identical.

There are 7 separate documents in this file. They need not be printed out, unless you choose to do so for your own reference. However, they must each be read VERY CAREFULLY. Please take the time to do so, as you will be signing your acknowledgement of the policies outlined therein. All student athletes and their parents are expected to understand and follow the guidelines set forth in these documents.
